What kind of help do we provide?
Laurels Home Care can assist you with the everyday activities you may have difficulty with. We provide a wide variety of support so you can continue living independently in your own home. We provide practical, social and personal support to people from the age of 18 onwards which will be tailored to your needs. Here are just a few examples of how we help our current clients:
Personal Care: Help with bathing and personal hygiene, assistance taking medication, assistance with eating and drinking, getting in and out of bed and a range of other personal tasks.
Social Care: Helping you maintain social contacts, companionship, escorting on outings and help with hobbies and pastimes.
Practical Care: Helping with housework like cleaning and laundry, shopping, lighting fires, meal preparation, collecting pensions and paying bills.
Long term or temporary respite care
We can provide a range of support for carers either on a temporary or long term basis. Our ‘respite care’ at home aims to relieve stress in families caring for a person who is affected by a physical disability or mental health problem.
How do I go about receiving home care services?
Our Care Manager or care co-ordinator will visit you to discuss what you need from us and they will prepare a personal care plan, matching your requirements to the home care supporter with the right skills for your needs.
Your initial assessment will:
- Concentrate on your needs
- Consider your views and wishes
- Focus on your strengths and abilities
- Identify any risks to you and the homecare supporter
- Help you make the right choice
We encourage your full involvement with the assessment to give an understanding of your needs and type of service you may require. The care manager will try to ensure that you are happy with the outcome of your assessment.
You and the Care Coordinator will discuss the outcome of the assessment. You may want to have a carer, relative, representative or advocate with you during this visit, so you are happy that you are in control of the assessment process.
Your personal service plan
If you would like to go ahead with the home care, a ‘personal service plan’ will be completed with you, outlining what help is needed and when it will be provided.
You will be notified when your care service will start and you will also be given the name of the homecare supporter who will deliver your care. Within 7 days of the commencement of service, you will be issued with a contract (if self funding).
Your follow up
After your home care service has been running for six weeks, your Care Coordinator will call to review your care package and ensure that you are happy with the service. Thereafter your care package will be reviewed annually unless your care needs change or you request a review at which point we will then carry out another assessment.
